History of Present Illness:

A 32-year-old male with no PMH presents to the ED for ~10 days of right eye pain   He does not wear contact lenses and denies discharge, change in vision or other complaints.  He has had pink eye before but it usually doesn’t last this long or become this painful.  The left eye is fine

Vital Signs & Physical Exam:

Vital signs are normal. Physical exam is otherwise normal except for the right eye, which is shown below.

Initial Differential Diagnosis:

  • Conjunctivitis
  • Keratitis
  • Iritis
  • Episcleritis
  • Sclertitis
